A fatal fail of type compliance breach triggers four downstream actions in QUALICORE:

The evaluation is locked at 0%.
An escalation is auto-raised on the supervisor's queue.
The supervisor is required to schedule a coaching session within 24 hours.
HR is notified if the agent has more than two fatal fails in a rolling 30-day window.
